Activity Request Form

An event cannot be added to the calendar unless it is approved by the Dean of Students and then sent to the Student Senate Vice President at least one week before the requested date. Dates, time, and/or location are first come, first serve. Organizations will be notified if a date, time, and/or location are not available and will be allowed to pick another date, time, and/or location.  All information given on this form must be factual and  true.

Once the Activity Request Form is completed, emailed to Aaron Quinn: (Dean of Students) and approved, the activity will be placed on the official activities calendar. If an organization chooses to cancel an event, they must notify the Student Senate Secretary via e-mail or phone 48 hours prior to the event.
