The Madog Center for Welsh Studies is building a biographical database on Welsh-Americans from all walks of life.  Please help us make it a successful project by filling out the following form and returning it to the Madog Center.  You may also send the information to us by either mail or by fax if that is more convenient for you.  Please provide the following information:

How many years of education do you have?  Please give names of schools, colleges, and universities.

Are you affiliated with a church?  If so, what denomination is it?  To what church did your ancestors belong?

What are your accomplishments?  What have you done in your life and in your 
community?  This can include anything from raising a family to activities outside 
the home.

Please feel free to include any other comments or history you would like to be
