Consideration for eligibility for Accessibility Services is open to any student (full-time, part-time, commuter or resident) who has a documented disability. Documentation of a disability may include a written statement from a physician or a psychologist depending the ADA referenced disability. In the case of a Learning Disability, a recent Multi-factored Evaluation or an Evaluation Team Report is preferred followed by an Individualized Education Plan from the referring High School.
Accommodation(s) are written after the Eligibility statement has been written.

I need to explain that the lower portion of the Intake form is used to update documentaion The update is important so that we can make sure that you get the right start, and in the right classes. All you need to do is to mark the line in front of the question that relates to your situation.
The Consent to Release form is to protect your rights under the ADA law that effects post secondary education. Please read carefully before you sign it. Please designate by writing in blanks with whom I can communicate. The consent form will require a signature to be activated.
